I think there are several abbreviations in there, but my best guess is:
Med[ical] cert[ifies] otitis media (dis[charge] wid/with deafness) NF [near field] vision wid/with strabismus ____

So this person had an ear infection that affected his/her hearing and had a vision problem as well.
