Help with name, company and regiment number

I would be grateful if someone can help me to complete the transcription of the attached line of writing in a WW1 hospital admission form. I read as follows:

Corps: R.S.C.H.T
Company: 66?
Regimental No: ?9856
Rank and Name: Dr   ???????????
Date: 15.9.17
Clark, Clarke, Batchelor, Diamond, Ruddick,
Yorkshire: Oaks, Denton, Sykes

Army Service Corps Horse Transport and his rank was Driver

The last digit of the company and the first of the regimental number look the same, so 39856?

Meaburn, Mennile/Meynell, Metcalf(e), Palliser, Robinson, Rutter, Shipley, Stow, Wilkinson

I've looked more at the Coy and Regiment numbers and I think I see a 2 rather than a 3. I see that there is also a 662 transport company on the Long Long Trail web page. I see the name starting with a "C" and ending with "ire". As for the middle I have no clue. I see the initial as M.  Does anyone have any thoughts.

To me the Dr's name is something like Chammiske or Cammiske, Cammick, Cammisk, Charrisk, Charrick. Garrick?

The Reg No looks to me like 29856.
