Frensham Hill makes sense as he had the accident at Bordon which is only 6 miles away.  Being taken to West Derby for burial also makes sense as he came from that neck of the woods.

Fazackerley looks like an error on the part of the contributor to Findagrave.

Update.  There wasn't, as I thought yesterday, a 17 Reserve Brigade.  The 7 Reserve Brigade RFA was at Bordon (where he had the accident) from October 1916 and it would make sense for someone sent back from France to be with such a brigade later.  The D Battery is still a puzzle, neither 1/4th or 2/4th had a D Battery but without his records we can't get any further with that.
