According to the BBC the police will get the data from the NHS Contract & Trace scheme, not the mobile phone app:

'Police will not have access to data from the NHS Covid-19 app. The app is anonymous so the government does not know who has been sent instructions to self-isolate'.

However, the British Medical Association said it was worried police involvement might put people off being tested.

Will this be the same police who are too busy to investigate burglaries etc even in the best of times?

UK Public Health Authorities have already been trying to find many of those who had to isolate for 14 days when returning from abroad.

Coronavirus (COVID-19) listed as a notifiable disease - GOV.UK
5th March 2020
At 6.15pm on 5 March 2020, a statutory instrument was made into law that adds COVID-19 to the list of notifiable diseases and SARS-COV-2 to the list of notifiable causative agents.

This change was made by adding them to the Health Protection (Notification) Regulations 2010.

This change in law requires GPs to report all cases of COVID-19 to Public Health England.

It's a bit of a joke classifying covid as a notifiable disease when the track & trace (how many billions was spaffed up that particular wall?) is so useless that if it does actually identify contacts, it's about ten days after they have been in contact with someone with covid.  So by the time they have been notified,  they've been roaming freely for longer than the illness lasts with most people.  However in the meantime they will likely  have infected the grandparents, the kids, their teachers, the workers at the supermarket and the jolly guy at the paper shop.  And they, not realising their peril, will have been going about their lives, unaware that all is not well.

The problem really is the notifiable disease legislation only puts the onus on the doctor to notify the relevant authority their patient has the disease.
I would also point out that covid-19 is no longer classed as a  high consequence infectious disease, that classification was removed on the 19th of March.

NOID, Covid-19
Coronavirus Covid-19 is an infectious disease, that is why we have Hands, Face, Space and Restrictions.

SARS-CoV-2 (causal / causative agent) / Covid-19 is a NOID a Notification of infectious diseases. Nothing to countermand or revoke the 5th March 2020 Statutory Instrument (S.I.) instruction (UK).
