Please could you give me your interpretation of the maiden name on this birth cert of Patrick Gavan.

That’s interesting isn’t it.
It clearly says Servant.
But no father listed. I am inclined to think the answer “servant” was in reply to a question about occupation.

I agree that it says 'formerly SERVANT', most likely an occupation rather than name
