Help with deciphering service record

Hi all,

I was wondering if anyone could help with deciphering what is on this service record?
I've managed to make out bits and bobs, but the writing is very faded and difficult to read, so can't get the whole story.

Any help would be greatly appreciated

Not entirely legible as you have seen, this is part of his medical history recording admissions (Adm) for two lots of dental treatment and two other hospital admissions.  There are other records on Findmypast which show his service in UK with the Leicesters Dec 1915 to May 1916 then transfer and service in France with the Machine Gun Corps until May 1917 when back to UK until discharge in Mar 1919 disabled with disordered action of the heart.
