Change of Name Code

I've just discovered from old NHS records that my birth was probably re-registered at some point in my early childhood when my father returned from sea. There is a record stating a change in my surname when I was almost five years old. The NHS used the code "CR 283" - can anyone tell me what this means please?

I am unsure whether the date given as the change in my surname is when my surname was changed or when the NHS discovered my surname had been changed. There is a discrepancy with the date of birth on my certificate and that recorded on the NHS records. This may be a transcription error of the NHS staff at the time they noted the change. Can anyone advise me on how I can double check when my name was changed? With regard to the date of birth - I was born in a hospital would they have recorded my birth date? Thanks.

I would assume hospital records will still hold details of your birth but whether they would give details directly to you I am not sure, I wanted hospital records for 1942 and I had to send my birth certificate and my parents marriage certificate, I was a twin and they would only give details of me, not my brother

If you look at your registered birth you can double check your mother's maiden name, see what you think, don't know what date

I have no idea what the CR283 means

CR283 is a form used when there is a change of forename, surname or date of birth.

I would suggest that you look for your birth details under what I assume are going to be two surnames.  You should have some ideas as to the dates involved.

If your birth was re-registered, then there will be a second birth registration in the indexes in the year when it occurred. You can order a copy of a certificate form that entry (or from the original first entry), from the relevant registration office or GRO.

My niece had 2 children before she married their father, she re registered the children into her married name some 8 years or so after, even though they were originally registered into that name, was it a possibility this person was born out of wedlock but both registrations should be listed
