It is possible that the hospital undertook the registration responsibilities, but perhaps they sought the family details from Elizabeth or from the older children.

The headings on Vic Death BDM records include

 Date and place of death;

 occupation of the deceased;
 sex and age;
 cause of death,
 duration of illness,
 medical attendant by whom certified and when he last saw deceased;
 name and surname of parents (if known) including mother's maiden surname        signature, description and address of the person who gave the information;  signature of deputy registrar, date and where registered,

 undertaker whom certified;
 name and religion of Minister, or names of witnesses of burial;
 place of birth of the deceased and how long he or she resided in the Australian colonies or states (stating which),
 name of spouse,
 place of marriage,
 age at marriage;
 names and ages of children of the deceased.

Do you have the historic image of the  1868 death registration?  What is recorded on it?  Does it give you an answer to the question "how long in the colonies?"

Sixteen years in the colony and Thomas' age is given as 15.  I think it is most likely that Thomas was born either onboard or soon after arrival, prior to civil registration.

I also think that Elizabeth's mother Margaret may have remarried to John STEWART in 1831 which would explain the use of both the BROWN and STEWART names by Elizabeth.

I found an image of the death registration for Elizabeth in 1911 in Victoria,

She was recorded as born Glasgow and 59 years in the colony (of Victoria).  She died 22 March 1911.  The informant was the Undertaker, and the cert is quite detailed.

1911- 59 = arrival 1852ish.

Thomas SMITH is buried at Fawkner Memorial Park with his mother Elizabeth James SMITH.

He died at Errol Street East Brunswick Victoria in 1929 and the informant stated his age to be 76 years.

He had shared the address with his mother at her death in 1911.

His mother left a will which favoured him and three of his siblings.

It is fine to write  one reply that addresses the main points of the new incoming information and any arising queries from it.
