could someone explain birth reg entry.

Ann Smith , Jones,  york 10a 111

but I have found a birth reg with same date , numbers, christian name ,
only difference is surnames ,
they are both next to each other in entry.

why would this be like that .   thank you .

Mother was widowed/ divorced.?

Usually you see

Speed (name) Smith
Jones ( name) Smith

Or us that what you meant to write

the first surname is correct , the second is not ?

Birth register entries don't show a surname for the child in England/Wales (until 1969), so are always indexed under the surnames of one (or both) parent(s) depending on their marital status.

A birth being indexed under two surnames would normally indicate an unmarried couple, but where both are named on the register entry, although there are other possible explanations and you can't be certain from the index alone.

(note that the old paper indexes, as seen on Ancestry, FindMyPast, FreeBMD etc. and the "new" GRO on-line index use slightly different indexing rules so results can differ, especially in this type of situation).
