Asylum Case Notes 5

These are the last of the case notes, and are much easier to read generally, but there are still some entries I can't decipher!  ???

Here's what I have so far:
15 7 39 Heart normal, lungs normal, abdomen? normal, weight 8 st, 7? lb
23.11.39 Cut? [?]
8.1.1940 Chronic left ear discharge. Continue H202 wash.
19.1.1940 Bruise on nose; struck by H. [?]
5.3. 1940 Physical exam [?]
Nothing to add to the above; health feeble, no mental changes.
END OF FIRST ENTRY
BEGINNING OF SECOND ENTRY
8.5.1940: cut on left side of fore-head; a violent? (or evident?) fall in day room
29.8.1940: mental note: a noisy patient, faulty in habits (W+D, don't know what this means!), not able to employ himself usefully, no mental change.
22.10.1940: abrasion over left eye, cause unknown
16.11.1940: abrasion on forehead; fell when walking amongst? bed
2.12.1940: old? cut opened by a blow from ?
21.1.1941 (or 4.1.1941?) 9.50 am, reported that he had an epileptiform? attack during the night in wd 13. Very ? this am and will not help. Refuses exam. [?] and see again at 10.30 am.


As always, any help is much appreciated.

I wonder if (W+D) could be an abbreviation of washing and dressing.

19.1.1940 Bruise on nose; struck by H. Daulby
5.3. 1940 Physical exam W[eigh]t 7st 7lbs

8.5.1940 - accident

29.8.1940 - W+D may be Wet & Dirty (as in Case Notes 3)

2.12.1940 - agree with Old cut... blow from Wm Cokersal/Cokersol

4.1.1941 (pretty sure it's 4, not 21) - epileptiform looks right.... V. resistive
The R with a crossing through it - I think I've seen it before in a medical context: Recommend? Request? Require?

yes, google confirms there is such a word as epileptiform  :D
