Could you pl let us know which particular sections you are having problems with so we can concentrate on those.
The writing is comparatively clear and so you will undoubtedly have managed to transcribe parts.  Some like the signatures are repeats.

then these calcs are set out.

Losing all prior service has implications for pension calculations. 

Sad tale all round.
