Query re a struck through name on death record

Hello everyone,

My query relates to an England and Wales Death Record.  The woman that it relates to was a Mrs. X with the maiden surname of Reilly.  Her married and maiden surnames are correct on this record but written under Reilly is another surname "Brady" which has been struck through and the notation (F) written beside it.

99.9% of my genealogy research is Irish based so I'm not too familiar with UK records.  To the best of my knowledge Mrs X nee Reilly was only married once. 

Was the surname Brady inserted into this record in error and the strikethorough and note indicate that or was she also known by the surname Brady?

Is there anything in the margin - initials or a signature?
I think errors are numbered and annotated  ???
