Relationship is brother to deceased so who was the deceased?  If male - will be same surname.  If female will be her maiden name

Can we have a date for the death certificate? There was a Samuel McIlveen in Matilda Street at 1911. Not of 159, however number on census did not necessarily match street number, but instead could have been order in which property visited.
