Asylum case notes

Can anyone help me read these asylum case notes, please? Any help is much appreciated!  :)

Here's what I have so far:

First image:
18 [referring to 18 Sep 1903] she shows no improvement mentally, is dull and confused [?]

25. She [remains?] dull incoherent and can give [not?] little account of herself. Fair health.

Oct 1. She has [improved?] of late [?] [quite?] [?] dull and demented. Fair health.

Nov 1. She shows no further improvement mentally. She is dull weak minded [incoherent?] and cannot give [?] account of herself. Fair health.

Dec 1. She shows no mental improvement. Is dull weak minded. No memory. Fair health

Jan 1. She remains in the same mental [?]. She is still dull weak minded [?] is at present [?] [?] with gastric pain. Fair health.

Second image:
Feb 1. She remains [??]... herself in the wards. Fair health.

Mar 14. She is noisy and excited and talks incoherently. Her memory is impaired and she cannot give a good account of herself.

Apr 1. She shows no mental improvement. She is [noisy?] and incoherent still. Fair health.

May 1. She shows no mental improvement. She is [? ?] and incoherent. Fair health.

June 8. She is dull and [?] and cannot give any account of herself. Fair health.

25. She [remains?] dull incoherent and can give but little account of herself. Fair health. . . .

Oct 1. She she has improved of late not being quite so dull and demented . . .
Fair health.

Nov 1. She shows no further improvement mentally. She is dull weak minded incoherent and cannot give much account of herself. Fair health.

Dec 1. She shows no mental improvement. Is dull weak minded. No memory. Fair health

Jan 1. She remains in the same mental state. She is still dull weak minded and has no memory. is at present confined to bed with gastric pain. Fair health.

Second image:
Feb 1. She remains rather incoherent and deluded. and refuses to occupy herself in the wards. Fair health.

Mar 14. She is noisy and excited and talks incoherently. Her memory is impaired and she cannot give a good account of herself.

Apr 1. She shows no mental improvement. She is noisy and incoherent still. Fair health.

May 1. She shows no mental improvement. She is noisy restless and incoherent. Fair health.

June 8. She is dull and taciturn and cannot give any account of herself. Fair health.

Second image, Feb 1: ... refuses to employ herself in the wards. (More or less the same meaning as Hollander suggested, but I think it fits the doctor's writing better.)
