Thanks Merlin the first image I recognise the form but the typewritten version I have not seen before could you tell me what it is called and who would have typed it someone with the shipping company or a clerk on his arrival?
Ruskie I take your point that it was the first form he signed so logically it is the right way.  I spent a lot of my youth with his granddaughter and I am sure if there had been a conscious name change there would have been a story she would have passed down.
I think my next move must be to get a transcript of his death certificate and confirm his birth details.
Thanks for all the interest.

Thank you for all your great messages.
To Alison2763 I don't as yet have Thomas COHEN's death certificate but on his son Mark Last COHEN's christening certificate his father is listed as a butcher.
To majm I will endeavour to get a copy of the original handwritten shipping records and check it out.
The family did reside in Balmain for quite some time and I have quite a lot of bdm records in that area all using the spelling COHEN.
To sparrett as I have heard COWEN is pronounced Co-en I thought Thomas may have got sick of spelling his name for people and let it change to Cohen.
My first adventure into RootsChat has been very interesting and will continue submitting.
Thank you
