Birth Certificate Information

On a birth certificate from 1886, the format for the mother's information is name, surname and maiden surname of mother.

If the mother had been married previously and then widowed, would her previous married name be noted too?

If the answer is "no" is it likely to be noted in the Parish Register?

Hi, my G-Grandmother was born Mary Fitzgerald

When she married for the 2nd time to a Colbert  (after a widowed "Flynn" marriage) her name is written as

"Mary Flynn" (formerly Fitzgerald)
