Can anyone help with a couple of words I can't make out in this letter which is among the papers for a discharge from the army.

 'With reference to the enquiry of the Major General as to the Medical Certificate in the ------ ----- document of Sergeant George Jerome. I beg to state that having no form to guide me as to the Discharge by Special Authority, I merely followed the usual form of Invaliding ------- to the latest instructions of the Director General Medical Department on these ------ '

I read the three words/phrases as:


discharge document

agreeably

on that head  - not on these ....
