Statement of service help please

Hello to all,
Could some one have a go at deciphering the chicken scribble on this statement of service please.
I tried to have more of the document in this post but it kept saying error to large, this is all it would allow.

I can make out some numbers but not in the real messy part.

First line is 12 Lt Dragoons

It would help if you could tell us whose record this is then we could look it up for ourselves and see the whole page.

Close enough, I think that the mess comes from trying to write over mistakes, but the months don't fit to the time line I'm working on so not the record I need.
