The form says born Portadoown [Portadown] not Porterstwon but birth registration gives place as Mulladry (not far from Portadown)-

Daughter Mildred Hewitt born about 1920 married James McLaren Jack - (the obituary mentions deceased sisters)

Obituary Vancouver Sun.................

Mildred passed away peacefully at the Brock Fahrni Pavillion in Vancouver, British Columbia, on December 11, 2009 at the age of 89 years.  Mildred was predeceased by her loving husband James and her two sisters Elinor Kerry and Pearle Cree as well as her granddaughter Amanda Jack.
