Deciphering Doctor's Writing

Hi guys, the ever challenging task of deciphering a doctor's scrawl. I can read part of it but there are words that I can't make out so can't get the full meaning. It would be lovely if some one could work out what it says.

After having the worst handwriting at school three years running, I can actually read some of this.
Sounds like some kind of psychiatric or competency assessment.


Can answer questions, carry on conversation.  Takes an interest in his surroundings, understands time and place.  Good memory.  Gives a full account of what he has been/seen.

States that his father forged document to .  That he worked with police sergeant.  States that police have a down on him.

Eating well, sleeping well.  Working in with

Yes, I've still got appalling handwriting but I just couldn't work out all the words, enough to make sense of it anyway. Yes, that one word you couldn't get - it's the crux of it, hopefully someone else can tease out the meaning of that squiggle.

Yes, that would make sense. It's that bit about his father that really interests me but the scrawl is particularly bad just there!

Might it be "States that his father forged document to home" - that is, the home or institution at which this document originated?
