The Common Room / DNA report presentation

I'm looking to create a simple report/presentation/booklet of my DNA results via Canva that can explain the results in a clear and simple way as I don't want to just copy the results off my Ancestry profile. I've tried all the usual places and not really seen any examples that others have done. Does anyone have any links or suggestions of places to look that I can use for inspiration?

From this part of my relatives Service Record I can see that he was severely injured on 12 Jul 1917 and admitted to adm56 General Hospital Etaples. Then there are the numbers 9 7/17. These numbers also appear on his SB36 pension card and seem to be a code for his wounds. Is there anywhere that  gives an explanation as to what these could mean, and how to decipher them?

I'm trying to read the name of this church, but can't find anything that matches close to the name in any lists of local churches. Marriage was registered in the City of Westminster.
The certificate says: Marriage solemnized at the Parish Church of St G****? Purlieu? in the County of Middlesex.

This is part of the Royal Marines Description Books (ADM158), and written in the Observations column is the word that looks like yd/jd/fd or sd? I can't work out what it is short for. It occurs alot in entries for different servicemen. Any ideas?
