The name in the margin is Johannes Saam (a man's name!) and the date 24 August 1853.
My Latin is not much cop, but it appears to be the birth and baptism record "in the Year of Our Lord one thousand eight hundred and fifty three, on the twenty-fourth of August" of Johannes (Ioannes in Latin) son of Wilhelm Saam and Anna Strohmaier. Godfather was Ioannes Iacob (Johannes Jakob).
Aha! Just found him on familysearch.org - born 24/8/1853 and christened on 28/8 at St Dreifaltigkeit (Holy Trinity Catholic Church), Frankenthal, Pfalz, Germany.
