A thought to consider.

If ELPHICK was not legally married to the mother of these children, the children would possibly be registered in her name, not his.

Nor would there be a death registered for the woman in the ELPHICK name and obviously not a marriage, 

Alternatively, consider  the possibility that children were not his (maybe some were, others not) and this would account for the lack of documentation in the ELPHICK name for the couple.

If our reading is correct, we must remember that the remarks made by the groom on the 1889 certificate may not be accurate.

No-one actually checked such information.

Records show towards the end of her life, the single woman who died at Warrington Hospital according to the online tree, did not have a complete birthdate known.

I agree with mckha489, she is not likely to be the missing wife.

Just mentioning also there is not a newspaper I have located named the Healesville Gazette on Trove.
I do see the Healesville Guardian, but digitisation does not seem to include the year 1883.

Do you have actual sightings of Alexander IRWIN in Victoria?

I cannot see an immigration in 1861 per Lightning as you have suggested for Sarah IRWIN, though there is one in 1863
Her death notice suggests 1861 arrival....??
The lass on Lightning 1863,is English and b. 1838.
