I received a response from FAAM, basically saying that it was probably a clerical error putting "S" for self in column 5.  There is no other way (other than that MMR ledger entry) to find out if the medals were issued or to whom they were issued. 
The email went on to quote the following:

"Escorting a hospital ship into Le Havre, at 5.25am, just as she reached the Whistle buoy at the harbour entrance there was a large explosion under the bridge. Her back was broken and she split in half, the fore part capsizing and foundering immediately, the after section floating for about about seven minutes before sinking. Several local boats closed the area and picked up survivors; One officer and sixty-one men lost their lives. The cause was probably a mine, laid by UC-25." (Source - "British Warship Losses in the Ironclad Era 1860-1919" by David Hepper).  The Admiralty papers on the sinking are in the UK National Archives, Kew, under reference ADM 137/3257 (not on-line).

HI Ady:
Looked at the FAAM website.  They specifically say that they don't answer inquiries by email but I wrote anyway and laid out the problem.  We'll see what happens.

Recently started researching family.  My Uncle...Llewellyn Arnold (Mercantile Marine Reserve), was killed on May 2 1917 aboard HMS Derwent, which was sunk by a marine mine off the coast of La Havre.
I have found his medal entitlement index card (See attached image).  I have been told by Nat Arch researcher that the "S" in column 5 stands for Self in which the medals were to be issued to the entitled recipient.  Llewellyn was listed as "Body Not Recovered for Burial", and, the researcher goes on to say, "I note that your uncle died during the War itself  and so could not have received the medals himself.  But Column 5 in the Naval Medal Roll in the record series ADM 171 covers ‘to whom the medals were sent’:
 
Roll:  M.M.R. FOUND in COLUMN 5
 
S. Self (ie the recipient)
FR. Father
MR. Mother
SR. Sister
BR. Brother
W. Wife
Ww. Widow
DR. Daughter
GODMR. Godmother
GODFR. Godfather

UNIV.LEG: Universal Legatee
except there is no listing to whom the medals were sent.

1) Is there any way I can find out if the medals were actually issued and to whom, and 2) Is there an official record that I can read which will tell me exactly what happened to HMS Derwent?
