Occupation Interests / Harmly's Idiot?

I've been researching my GUEST family and have come across them on 1851 and 1861 censuses in Christleton, Cheshire.
One of the occupations of a relative is "Harmly's Idiot" for both of those censuses.
(1861 - RG9/2628; Folio 125 Page 13 )
Also, in the 1851 census, in the "Relation to Head of Family" column it says that the youngest person in the GUEST household that they're a "nurse child".
(1851 HO107/2171 Folio 420 Page 8.)
This person, George Guest, shares the same surname as the head of the family.
What do both of these things mean?
