application for certificates

Can anyone tell me how to apply for a birth certificate for someone born in St Helier Jersey in 1851?  To whom do I apply, address etc, how much should I send and to whom should I make the cheque to?  I have name and date of birth and name of mother and place of birth so do not need a search doing.

Many thanks for info - I had found that site but as it was last updated in 2001 and as the rates for getting certificates have gone up in the uk since then I would like to check out that they are still accurate.  regards  Janet.
