What should I do with this photo to fix it? It is my brother and the only photo I have of him
Advice and help welcome.

This only my humble opinion (no doubt others will know better):

I would crop in close to the figure, as long as the back ground is not important, this will reduce the amount of work.

Then convert to black and white. 32 bit if you can.

You should then clone in the white bits, maybe use remove dust  and scratches filter.

Then try a low level of sharpen.

Holy Smoke Roadrunner......If it was me I would just crop your Bro' out of the Picture and then attempt to just repair him.....even if it was only from the waist up....

The trouble with a lot of Photos isn't just the fact that they get mangled a bit......you also have to take into account the Camera and film used at the time plus whether the cameraman/Woman had the shakes or not!!

For the most part, the older the piccy the better, because they had to stand perfectly still (usually supported and in a Studio), unsmiling, for quite a while and so blurring etc., was rarely a problem.....

I will try following advice that both of you gave. My mother just passed away and we found this photo stashed in a box which she has kept for at least 45 years, so not in that bad a shape.
I will try and see what I can come up with.

So sorry about your mum.

Have had a little play with your pic. Have removed his overcoat. I thought the background added to it so left it in. Not brilliant, but the best I can do in the short term.
