Last year, when I still hadn't learnt the importance of keeping EVERY source I found a link to a useful resource.

This was a  template for Certificate Order Form.  Not the one sent to the GRO but one I keep at the front of a file and any possible certificate that I find I list on here.  It allows to me to state if/when ordered and whether useful (ie correct one) or not. 

Unfortunately I foolishly used my master copy and now can't find the original source.  Can anyone help ??
HAMPSHIRE - Macdonald, Matthews. 
DEVON - Bennett, Macdonald. 
SOMERSET - Macdonald. 
SURREY - Ayling, Ansell, Collins, Matthews, Macdonald.
SUSSEX - Collins, Matthews. 
CUMBERLAND - Grainger.

Sorry can't help you locate your form but have you tried to recreate it using Excel.

You can them keep your master copy on your PC
