Which forms - WWII Naval Service Records

My father died recently.

My mother and I have never known anything much with regards to his service when he was called up to the RN in 1944.  We have chatted about this over the past couple of months when we come across pictures and things. We have therefore taken the decision to ask for his service records. However, these records are the first I have ever asked for.  I have downloaded the SARS form and a Certificate of Kinship form from the Veterans site.  I can be slow on the up take but do BOTH forms have to be submitted for the same set of records?

Can someone please expand on all this form filling for me??

Yes Jude you need to fill in and return both forms.

The Certificate of kinship aims to identify you as the next of kin, while the SARs is for access to the record.
