lunatic asylum registers

Hi, i wonder if anyone can help.  i spent a good few hours searching through asylum registers at TNA and have found a possible for my ancestor.  I have two queries:

1.  does anyone know what these column heading means? 
the first one i think is recovered, but the next two, are they released and improved?  this is just my guess??




2.  on the second photo, in the middle is the name of an asylum but it is smudged, any ideas what it says?    ( next to the number 16!)

Hi Sara,
It's certainly not easy to decipher - might be an abbreviation, or just bad spelling!

You might find it interesting anyway, Sara.
I'll look through the list, too, but I thought two pairs of eyes (or more) might be better than one.

I think its abrreviated (as in Oxford and glamorgan below it)
starts with an s and perhaps ends with one too, with maybe a th in the middle...
