I see that he is commemorated on the Portsmouth Naval Memorial (Panel 80, Column 1).

Do you have a photograph of that?

If not, I can see what I can do when I'm next off work.

Thanks again to everyone for such amazing help. 

I contacted the Submarine Museum yesterday, by email not expecting a reply for a week at least.  And they came back within a couple of hours with lots of information (as per Scrimnet's posting) but also said they had a photograph of Lt Harold Cook and a second photo of all the Chariotees which are available for purchase.  Needless to say I'll be buying a copy of these later today.

Thank you for my acceptance in this chat. I have just read a book about HMS Thunderbolt that was the carrier of the human torpedo for the raid in Palermo port. The raid was a success but the two seamen on the torpedo lost their lives. Here in Palermo, on the wall of Molo Bersagliere many years ago was placed a tablet in memory of the courage of the two seamen.
