I found a Thomas Blezard who was also in Royal Lancaster Regiment ( maybe that's the key !!)  but no address I'm afraid  :-\

Found Norman Whalley on CWGC as Whelley so possible scanning error ....maybe need more proof before they can change it(if its wrong and SDGW and memorial are wrong)
