not sure about surname - it looks like a date - Spring 1904  :-\

I'm at work at the moment so can't look at my handwriting chart but I'm pretty sure it's not a capital A and is rather an R.

My instant recation, before I read any other replies was Rex Spring 1904, so am agreeing with Gadget and Kerry.
