I don't think it would.  I've seen them with spaces for information on spouses and children, but I don't think the ones from that time frame had a space for parents' information.

Yes, it shows the place of birth.  You should be able to see an example of one here:  http://www.footnote.com/image/#9711255

There's information on obtaining a copy:  http://www.archives.gov/northeast/nyc/finding-aids/naturalization-records.html#copies  The court where his information is from is the US District Court for the Southern District of New York.  There's a printable form to send in here:  http://www.italiangen.org/Southern.stm

Interesting document.  I hadn’t realized that the naturalization applications contained so much detail.  My own ancestors arrived much earlier and, with one exception, I don’t know if they bothered to become naturalized citizens.  I did find, in a file of old papers that my father had stashed away, a copy of a naturalization application for my gg grandfather in 1855.  It contained almost no useful information but was a charming document nonetheless.  In Marquette County, Wisconsin in 1855, they had no standard printed up application forms and so they had made up their own hand lettered form, leaving spaces for the applicant to fill in his data.

I was able to find my grandfather's petition for naturalization on Ancestry, so I thought I'd look for Derek Jacob Tammenga's petition. His petition also seems to be on Ancestry.

Under the heading "Index to Petitions for Naturalization filed in New York City, 1792-1989"

There is also a Jan Otwald Tammenga, born Mar 25, 1926, naturalized Apr. 4, 1955, Petition # 655893.

You can also contact the U.S. National Archives regional office in New York City to get a copy of his petition. The toll-free telephone number works only from the U.S. and Canada. The National Archives, in its wisdom, may be proposing soon to shut down the New York office. You should contact the folks there quickly.
