Jim dated this as 1890s and if I'm correct they weren't using head supports by this date. 

The youngest child is also holding out her legs/feet and these aren't at all blurred.

On closer inspection it looks like the older girl seated...could have had her head superimposed into the photo...no evidence of the others being added later that I can detect...the little girl's head also looks odd but they don't have much neck at this age.

I wouldn't say she was 'holding them out' - the fur rug is obviously hiding something.  Also the older girl's dress seems to be draped rather too widely ?

Some photographers  had boards with a hole in them for the subjects head to fit through, my father had one taken when he was in Edinburgh in 1939/40,   The board was a scottish soldier in traditional Scots kilt and all the trimmings.

I have never seen it with five children though.

Had another look at this & agree her head has been added.I have seen similar in others.
For whatever reason her head has become blurred or distorted & rather than get 5 kids
togged up & marched back to the studio (to the annoyance of the parents no doubt) the
studio has taken a corrective photo of just the girl.MM's correct that posing stands weren't
used by this time & hadn't been for around 20 years.
The draping of her skirt may be just that the chair's a bit tatty or the photographer was getting glare from it.
I've resized her head slightly to what I think it should be.

I'll disagree on this one jim, maid marion, et al, I think it's the flash effect combined with a slightly out of focused background, hair oil and pale faces. The dress is because of the seat width and the little one has her calves on the seat, hence her legs as they are.

If you zoom in on this photo...the girl's head is pixelated where as the other heads aren't...this leads me to believe that her head as been replaced and   resized to fit the body...Just my opinion...For what it's worth.
