**COMPLETED** German Geburtseintrages Deciphering Please

Looking for help deciphering (or translating) the following birth record.  I tried to only include the parts I couldn't figure out on my own  :)

A little background - Maria (mother) had 4 illegitimate children before this son.  Church records did not list their father.  Does this record state Mr. Leon was the father or did he just record the birth?  He had been married to Maria's mother (who passed away before this son was born).

Looking (in Germany) for Matz & Scherf/Schrift families near Forkenbeck/Ronkendorf (now in Mecklenburg-Vorpommern).  Children born 1850's, married in Germany then came to US in 1882 & 1883

3rd paragraph:

The (person's name) declared that he was present at the delivery of a child of Maria

Last line is a piece of "officialese": "Vorgelesen, genehmigt und untershrieben"
Read out, approved and signed
