Statement of Services (Army Form B200) 1914-1918

Does anyone know what Army Form B200 looks like. I have viewed my late grandfather's service record from the First World War but because a lot of the documents were burnt around the edges (and the titles therefore missing) I have been unable to distinguish them from one another. Is this the document that outlines a soldiers change of regiment and recruitment dates etc?

If you Google "World War 1 Service records - statement of service" and then click on Images - you should find at least one example

Thank you so much for the "Long Trails" site. I cannot stop reading it the information has answered so many questions I could not find info on WW1 and, I believe, I may find answers on The Royal Waggon Train that so far have eluded me.  Lookin2

Apologies for not replying earlier. At the moment I am chasing up several brick wall problems that I tend to get a bit distracted. Thank you for all feed back. I am pretty certain now that the form I have been looking at is indeed Army Form B200/B200A - Statement of Service.
