help with Ukraine locations and mothers' maiden names please

Marriage record 1916 from Vital Stats Website for Manitoba Canada:

locations of birth in Ukraine for bride and groom  (see beside their ages...i.e. 24 and 19 respectively)
and surnames of their mothers....lines 6 and 16 on attachment ( sorry, you'll have to count down )

I think the first birthplace is a spelling of Pilatkowce, Borszczow, Ukraine, and the second Kociubince, Husiatyn, Ukraine.
