Help with a death certificate

I have another Scottish death certificate this one is from 1921.
This one's hand writing is quite atrocious, I have made out what most of it says except for these three columns- When and Where Died, Cause of Death and Informant.
Hope some one can help make sense of this.

Chronic Valvular Heart Disease
5 yrs 9 mos. 15 days
Senile Decay
ArterioSclerosis

The time of death is a bit tricky, but I think it is "4h. 30 m. PM".

Usual residence was "13 Waverley Street, Falkirk".

Informant - possibly R. G. Nelson?

Freckneale, thanks wasn't completely worried so much about the informant or exact time of death so don't worry about it   :)
