How do I fill in the form ??
I have the microfilm number. That's ok

But :
- What is Item Number/Call Number ?
- What is Parish and Volume Number ?
- Registration, page number or item number ??

Blue70 has found the image online for the record you are looking for but in answer to your question.  When I have applied for these records I have ignored those questions if the answer was not obvious and have still had the copy sent.

Just recieved a reply to a request. The old form is about to be discontinued.

Dear Patron,

 We have posted at https://familysearch.org/learn/wiki/en/Photoduplication_Services the new form that has been put into use for requesting records.  The old form will no longer be acceptable.

I wouldn't mind but they have closed three FHC's near to me one of which is 3 miles away and the other two approx 25 miles away. Public transport would be a nightmare.  I wonder where they will relocate the London one, it was only supposed to be at Kew until early 2013.

I have just looked at the map of FHC's in the UK on their link and it could just eliminate everyone in the UK  ::)

I put my order in on 13th April and received the records yesterday, so it's taken just under 2 weeks.  How's that for service  :)

I sent off 5 requests 2 weeks ago. Received a reply that it would take 4 -6 weeks. Exactly 1 week to the day I had received my response(s) Very impressed & happy. ;D

Hopefully that quick turn around can last.
