Cemetery Name and Location

Below is a portion of a terrible copy of a 1968 death record I recently purchased from the Commonwealth of Pennsylvania.  I an having a very difficult time trying to determine the name of the cemetery and the location of the deceased.

The decease was buried either in Philadelphia or in one of the outlying counties surrounding the city.  Any help would be most welcomed!!

I think it's Gulf Mills - Gulph Mills on current maps. It's to the north west of Philadelphia near West Conshabooken.
