German translation please :)

I've just managed to get hold of some PoW paperwork for my great grandfather. One of those is written in German though... Given the context, I understand the gist of what is written but wanted an official translation :) Apologies for the quality of the image, not much I could do about that  :-\

I hope the following will help you, if not "official"  :)

1 Can hardly decipher first word, underneath is number (no.)

2a) Surname
2b) Forename (only the name by which he is usually known)
2c) Russian nationals only must give forename of father

3 Rank

4a) Troop
4b) Unit
4c) Company

5a)  Date of capture
5b)  Month? and day
5c)  Previous whereabouts

6a)  Date and place of birth
6b)  Next of kin
6c)  Relatives

It says "Münster II" and I assume that was the internment camp

some minor corrections.

First word is "Lfd" laufende - continuous numbers

5b  place (Ort) and day

6b and c  address of next of kin

Yes, Munster II was his last internment camp. I assume all this was written down when he was shipped over from Dulmen.
