Having trouble transcribing this surgeons remarks from civil war file.

Hello all,

Can anyone transcribe this?  I usually am pretty good at transcribing and reading old writing.  This one escapes me.  Any help you can provide would be greatly appreciated.

A start:
Right shoulder slightly smaller
Tender and limited in motion/movement
No other ...................... of rheumatism
Disease of heart
............... of dullness not
increased.  Apex .. is present at fifth intercostal
space just within ..... line.  Rhythym -
irregular beats ......................systolic
murmur ..............................hypertrophy
Considerable dyspnoea no ....  cyanosis

Thanks for the start.   I've been able to get a few words here and there, but nothing like what you came up with. 

Here's the best I have for all of the document (including your helpful insight of the first two paragraphs.

There's one record (the 1890 US Veterans Census) which said my ancestor had been shot in the hand.  However, I haven't found anything else which mentions this.  The last paragraph seems to have scars as the second word and the next line might have the word hands.  I'm hoping it might substantiate the 1890 census which indicated he was shot in the hand.

After cyanosis:

No abnormal sounds are heard over either lung.
Liver
Upper ?? with sixth rib,
lower with tenth and does not extend below the
ribs

Urinalysis ??? acid

(looks like) vicious habits
No scars on genitals or groins ?
inguinal glands, no subclinical tenderness

No other disability found to exist
? find that the aggregate permanent disability
for ?? support the manual labour is ?
rheumatism and ?of heart but due to
vicious habits and warrants a rate of ?? a month.

That writing is awful and I am happy to be corrected on the above if someone sees better than me!
I suspect that "vicious habits" implies that his problems were thought to be secondary to something like syphilis ---

I've been trying to work on the section on Kidneys:

Kidneys.
Urinary analysis. Color amber. ? acid.
Sp gr 1020. No albumen. No sugar. ? clear.

Not much help but it gets a few more words (I hope)  :P

Here's a bit more. But like Annie says I'm happy to be corrected. Terrible writing and I wonder if some of the words are abbreviations. The only word I can recognise easily is 'no'  :)

Think the final two sentences are "No mucus patches on bony ?muscles No inictions (indications (abbreviated)) of vicious habits"
