On 6 January 1816, at 10.00am, before me, Sunard(?), mayor of the town of Ouzilly, first arrondissement of Loudun, département of Vienne, carrying out the duties of Officer of Civil State, there appeared Urbin Assailly, who declared to me that Marthe, his wife, gave birth the previous day at 10.00pm to a male child, whose names are unknown. The declaration and presentation were made in the presence of Laurand Roussault, day-labourer, and Louis Vincent, shopkeeper, of this town; the said Hurbin and the witnesses stated that they were unable to sign.
ADDED - Or was the child named Urbin, after his father?

The annotation in the margin - Naissence de urbin assailly - might suggest that.
