My relative , what was his discharge papers saying .

EXTON, from Rutland, Stamford, Boston, Lincoln. LANES, from Coleby,to Bracebridge Lincoln.WAKEFIELD,PROUDMAN Cheshire and  Stafford.<br />PINDAR, MOORE, ,CHAMBERS mostly from Lincolnshire.
LAING from Elgin ,Scotland.
 HADDELSEY from Caistor,and Grimsby Lincolnshire.                   
 Parfitt, Le Gros ,Le Sueur, from Jersey.
Martin, from Doncaster  to whelyn garden city, London.
BINT, Worchester, in Australian mint.

The top line says (I think)
Haemoptysis Nov 1918 in hospital 3 weeks. (haemoptysis = coughing up blood)

Line 3 is Anaemia & haemoptysis

Line 4 is Occasional [???]

Agree with JenB that line 4 is Occasional
Can't make out the next word after that. 
Last word is fits.

nearest that I can get is occasional epileptic fits  - but epileptic wrongly spelled
