Inquest verdict

Please can you help in deciphering this verdict of an inquest.

We can make out
 Injuries accidentally ?  ?  ?  falling from a scaffolding at a building on church? of erection?

If that is for the outcome of an Inquest conducted in NSW, then the NSW death cert would likely have the same or very similar wording, but it would be in a different hand. 

The third line reads 'building on course of erection'  where 'on' is used to mean 'under the' . I agree with the other two lines already given.

OOPS,  ADD sorry 'in' course of erection...  'in' being 'under the'
