In 1911, 47 Church St in Padiham is occupied by a Doctor Grant-Jones ....he may have been running some sort of clinic there perhaps.

1911 electoral register

47 Church Street Padiham - John Grant Jones

The free index to the 1911 census shows John Grantjones to be a Doctor of Medicine - perhaps Samuel was taken there unwell and died on the premises ?

Thank you maybe that is the answer maybe he was taken ill and died at the doctors, he could have been visiting family in the Nelson Burnley area, and was on his way home and then taken ill because in those days,he would go through Padiham on his way back to Blackpool.
