my paternal great grandmothers brother- JACOB no surname on cert.

If you look at the column headings you will see that there is nowhere on the birth certificate for a child to be given a surname.  The indexing is usually done using the father's surname if he is named.

Wolitzsky is recorded as Hilda's second forename and her named father is recorded with a surname of NATHAN.

she wouldn't be the first woman to use names in such a way so as to cover up illegitimacy.
