Handwriting on death cert help

Hello,
I am having a bit of trouble understanding part of the attached certificate, I am getting some words, but not complete.  Is anybody able to assist on both cause of death and description.

                [doctor's initials]

Certificate received

Coroner for the City

Inquest held

Or instead of doctor's initials, perhaps PM for post-mortem. Doctors' initials aren't usual.

Not important, but the coroner’s surname is G. W. HUNTBACH.
