Ancestry's framework is based on cataloging rules for published items, i.e. books, pamphlets etc.  JohninSussex is correct in saying the item is not published.  It is made available as a document, so strictly falls under rules for description of archives, which are looser in format and adapt to the item.  Choose a sensible format and be consistent.

I use on PAF (Personal Ancestral File)

Source is GRO Online Index
Author is gro.gov.uk
Comments is (Birth and Death)
Citation Actual Text is
BIERTON, HANNAH       Mother's Maiden Surname: STRATTON
GRO Reference: 1859  S Quarter in LEIGHTON BUZZARD  Volume 03B  Page 403.

which shows on PAF as,
gro.gov.uk,  GRO Online Index.

"BIERTON, HANNAH       Mother's Maiden Surname: STRATTON

Any of my sources on Ancestry are there from PAF when I upload.

Call Number? What on earth is that?!

Rather old-fashioned.  Used in large libraries to "call" a book from a stack - when libraries had lots of staff.  Historically, books may not have been shelved in a browsable order by Dewey or Library of Congress classifications.  They were added to the shelves consecutively, so the call number identified the shelf and item number where they would be found.

Nowadays you might be able to put an internet reference in that box and it would serve the same purpose!

I've now bought quite a few of the £2.50 digital birth and death records from GRO.

If I add a 'fact' to my tree - such as a death - I would then attach one source and one media item.

The source would be Ancestry's own source citation from the England & Wales, Civil Registration Death Index, 1837-1915.

The media item would be the image I purchased.

One important thing is to make it clear in the citation whether you are referring to a certificate (or pdf/digital image) that you have or have seen, and therefore have evidence of the fact it is attached to, or whether you are citing an index entry alone (where you are making assumptions)
